A Clinical Study of Mesocaval Shunt Using Dacron Tube

Abstract
Three cases of side-to-side mesocaval shunt using woven dacron grafts in liver cirrhosis with massive
esophageal hemorrhage and intractable ascites were performed recently and here we report with some
literatural review.
We like to conclude that this procedure has four comparative advantages over other various portasys-
temic shints;
1) Side-to-side mesocaval shunt is a functional shunting rather than the mechanical.
2) The size of the superior mesenteric vein is approximately 9¡­11§® in diameter which is not
only sufficiently large enough for reduction of portal pressure but also small enough to prevent for
high-output congestive cardiac failure and post-shunt encephalopathy. Reduction of portal pressure in
our 3 cases were 157§® saline, 120§® saline and 110§® saline respectively.
3) Since the superior mesenteric vein is more distant from the liver and the heart than other veins
used for shunt, it has less chance of high output cardiac failure occuring secondary to hemodynamic
augmentation.
4) The operative procedure is much easire and the mortality is less.
